1. To obtain a medical certificate for conductor license in Madhya Pradesh the applicant has to approach the Chief Medical Officer (CMO).

4. The applicant should make sure they are eligible for applying by reading through the eligibility criteria that has been listed out under the “Eligibility” section of this page.

6. Application forms can be obtained from the appropriate RTO offices to which his area belongs to.

8. Go to a Chief Medical Officer (CMO) to examine
   
9. Medical officer will look in to the test results as required
