1. Go to the Municipality governorate branch concerned with the service - Permits for Municipal Services Department - Permits for Municipal Services Inspection - Health Certificate Section.
2. Submit all required documents.
3. Complete the Construction Department application form.
4. Proceed to the Inspection and Follow-up Department for checking the real estate and preparing a report.
5. Applicants will need to go to Construction Department to complete the form issued from the License Department, then recheck the License Department.
6. Applicants will receive a letter directed to Kuwait Fire Service ?Directorate for issuing fire extinguishment license from the Prevention Department
7. Visit the Department of Licenses again to have the store checked and ensure how far it confirms to the health terms and conditions
8. Pay the issuance fee and receive the store health license.

• Written letter from the applicant
• Copy of the lease contract or an equivalent
• Copy of the commercial license for the office of the store
