- To become a National Health Fund (NHF) Pharmacy Provider as an owner of a private-sector pharmacy or a duly authorised officer, you should submit your complete application to the NHF office providers relation unit. Please view the NHF contact details via this link: Link
- Obtain the providers application form either as for private sector or for public sector form the NHF officer or download it form their website:Link you can also download the form directly via these links: Provider form- Private sector: Link ,Provider form- Public sector:Link
- Complete the application form correctly and then obtain a character reference letter as then owner or director of the pharmacy form any of the following persons: Attorney-at-Law, Minister of Religion, Notary Public, Justice of the Peace, Superintendent of Police or higher rank
- Then prepare the rest of the required documents and attach them to the application form in a single file
- Visit the NHF office and submit your application to the NHF officer at the service desk
- The official will receive and forward to the necessary division form processing
- Then your application will be scrutinised by the relevant authority to determine if it meet the required standard for approval
- If your application is approved, you will be offered a Provider Agreement which will specify the Provider’s contractual obligations, operational procedures and requirements of the National Health Fund.

- Pharmacy Council Certificate of Registration of the Shop
- Pharmacy Council Certificate of Registration of the Registering Pharmacist
- Certificate of Business Registration -Registrar of Companies (if applicable)
- Certificate/Articles of Incorporation and Articles/Memorandum of Association
- TRN (Business)
- Government issued identification for all owners
- Letter indicating hours of Operations
- Current Police Record for owner(s)/director (s) of the pharmacy
- Character reference letter
