Apply In Person

1. To Apply for Delayed Registration (Late Entry) of Death, the applicant has to approach the Commissioner & Director of Municipal Administration, link
2. An application form will be provided to the applicant by the concerned officer.
3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
6. Submit all the documents with the completed form to the concerned officer.
7. The applicant has to pay the fees of Rs.500 or the prescribed fees as requested by the authority.
8. An acknowledgment receipt will be provided. Please keep it for future reference.
9. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
10. Once the process and the death record verification is completed with the death occurred hospital, the applicant will get SMS from the office for collecting the certificate.
11. The applicant has to visit the centre to get the death certificate.
12. The processing time for this procedure is up to 2 weeks.

• Any birth or death which has not been registered within one year of its occurrence shall be registered by an order of the Executive Magistrate not below the rank of a Revenue Divisional Officer and on payment of late fee.

• The Government of India had enacted the Registration of Births and Deaths Act in 1969 to promote the compulsory registration of births and deaths. The Act made registration of births, deaths and ‘stillbirths’ mandatory in India.
