1. You should visit the Caisse Nationale de Sécurité Sociale (CNSS) office near you and express your interest at the service desk.
2. Request form can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download :(Application form)
3. The request form should be fillled, signed and stamped by the employer, attach all the supporting documents (listed below) and submit them for verification and processing to the attending official at the CNSS.
4. If the authorized official is satisfied that all the documents are in order you will be notified of the outcome and be given further instruction.
