Apply In-Person:

1. The applicant should start by first visiting a police station to report the loss or damage of the certificate of registration.
2. After reporting the loss or damage you are also to acquire a police abstract from the police authorities which is a prerequisite to making the application.
3. To Replace Lost/ Damaged Community Based Organization Registration Certificate (CBO), the applicant has to approach Ministry of Labor and Social Affairs in Ethiopia contact link
4. Applicant has to get the application form to renewal from the respective office.
5. Applicant must provide all necessary information in the application form.
6. Make sure you have all the documents mentioned in “Required Documents” section of this page.
7. Complete the application form and attach the required document.
8. Submit the completed application form along with the required documents and abstract report to the authority.
9. Fee for this procedure has to paid as per the authority request.
10. Upon the successful registration, the renewal of license will be issued within 2days.
    

• A letter to the Registering Authority detailing the Reasons for seeking Replacement of the Certificate. The letter must be countersigned by the Assistant Chief /Chief of the Location/ Division or an official of the Social Development Committee.
• Minutes declaring the Loss and Reasons for replacement Endorsed by at least1/2 of the members must be Attached to the Application form
• A Police Abstract.
• Attachment of a portion of the original certificate/photocopy where Available
• Replacement fees receipt.
