Cyprus - Replace Lost / Stolen Civil Identity Card


Procedure

  1. To apply for a replacement card, applicants will need to submit their applications at the local offices of District Administration in Nicosia, Limassol, Famagusta, Larnaca and Paphos or the local Citizen Service Centres. To know more you may contact CRMD, contact numbers are provided below.
  2. Complete an application form and submit it together with all other required documents. (see list below)
  3. Pay the replacement fee





Required Documents

  • A completed relevant application form.
  • A 4 X 5 cm photograph, certified by the respective community president. In case the applicant presents himself personally the above certification is not necessary.
  • Original Birth Certificate.
  • Original Marriage Certificate in the case of a married woman who wants to change her surname or a Divorce Certificate in case of a divorced woman.
  • Passport in case the applicant has an additional nationality except for the Cypriot one
  • Additional documents in case of loss or theft of the Civil Identity Card:
    • Affidavit from Court regarding loss or theft of the Civil Identity Card
    • Written Complaint to Police reporting loss or theft of the Civil Identity Card

To apply for a replacement card, citizens should submit their applications at the local offices of District Administration in Nicosia, Limassol, Famagusta, Larnaca and Paphos or the local Citizen Service Centres.
