Procedure

Apply by Phone/Email/Fax

1. The applicant will have to contact the following email or phone to get a change of name package, find the contact details under the “Office locations and contacts” section of this page.
2. Here is the contact Link.
3. The authorities give further information.
4. Fill out the application form with relevant details.
5. Attach all required documents as mentioned under the “Required Documents” section of this page.
6. Submit the form along with documents.
7. The applicant will receive their documents within 4 weeks.

Required Documents

If the applicant wants to change their name to back to their birth name after a divorce or any other reason, they will need the following documents:

• Birth certificate
• Marriage certificate
• A divorce document
• Proof of current name such as:
  • Passport
  • Driver’s License
  • VAC card

Eligibility

• The applicant must be 16 years of age or older.
• The applicant must be a resident of Yukon.

Fees

• The applicant will need to pay a fee of 50$ to change their name.
• For each additional family member that is included in the application form, the applicant will need to make a fee payment of 17.50$.

Validity

• The document is valid forever and does not need to be renewed.

Instructions

• If the applicant is changing name after marriage they will only need to pay for the change of name certificate.
• If the applicant wants to change their identification they will need to contact the issuer to know the process.

• Reason for the proposed name change
