Walajah - Apply Identity Card for Differently Abled

Procedure[edit]


This procedure explains various ways to Apply Identity Card for Differently Abled in Walajah.

  1. Go to Government Peripheral Hospital in K.K.Nagar. Land Mark : Next to Udhyam Theater and near ESI Hospital. If you are from a different place or location, please contact your local district hospital who should be able to help you.
  2. Register your self to get the Disability form by saying the type of disability to the person issuing the form. Since it is "Q" basis be earlier. Time starts at 9.30 am till 4.00 pm.
  3. Buy Railway and Bus Concession form outside which is at the same place where xerox taken there.
  4. Fill all the form by your self hand written with 2 Passport Size Photo for Disability Certificate, 2 Passport Size Photo for Railway Concession and 2 Passport Size Photo for Bus Concession and a Aadhaar Copy with Ration Card. And Don't forget to sign these forms.
  5. After 9.30 am the staff will call you according to the "Q" basis. Then you must say your problem to the staff. After verification by the staff he will issue a passbook along with the document and forms given by you to visit the particular government doctor. The doctor verify you and sanction to issue the certificate's.
  6. You must submit the doctor verified copy to the staff. It will take 7 to 15 working days to receive the certificate once submitted. Please collect the date when get the certificate.
  7. When receiving the certificate to the given date please bring the same photo given at the time of submitting the form to identify your certificate along with the Aadhaar card and Ration Card.
  8. After receiving these certificate take a color copy for the certificate & passbook and submit it at back side of the office to get national wide UDID Card along with the Aadhaar Card and Ration Card. It will take 6 months to receive you UDID Card at home address specified in Aadhaar Card.
  9. To get railway concession download the from Application for issue of railway identity card for physically challenged persons for ticketing. and fill the form along with 2 Passport Size Photo.
    LIST OF DOCUMENTS TO BE ENCLOSED:
    • Railway Concessional certificate – copy – self attested
    • Disability Certificate(Medical certificate) – Copy – self attested
    • Date of Birth Proof (SSLC Certificate) - copy – self attested /
    • Voter ID – Copy – Self attested
    • Residence Proof (Ration Card/Telephone Bill) - copy – self attested
    • Any photo Identity Card issued by Govt. – PAN Card/Voter ID / Aadhaar - copy – self attested
    • Passport size Photo in colour – Two (with attached along with application form)
    • Disability Passbook first 2 page – Copy – Self attested
  10. Submit this details at DRM Office situated at MGR Chennai Central Local Train Ticket issuing office 4th floor through LIFT and get the acknowledgement copy. It will take around 30 days to 45 days to receive. You will get a call or go in person to get the card.

Note: The Director General of Health Services, Ministry of Health and Family Welfare will be the final authority to issue the ID card based on the documents.

Eligibility[edit]

Persons falling under the above categories with a minimum of 40% disability are eligible for entitlement of all benefits provided by the Government.



Fees[edit]

Explain the fees structure which is required for obtaining the certificate/document.



Validity[edit]

This card is valid for a period of five years from the date of issue. On expiry the card should be surrendered to the issuing authority and an application for issuing a new card should be submitted if required.

In case the card is lost, new card will be issued on the payment of a penalty of Rs. 15/-

Processing Time[edit]

Identity cards will be issued between the period of one week to one month from the date of application with the requisite certification.

Instructions[edit]

  • Identity cards for Person Differently Abled are issued by District Differently Abled Welfare Officers based on the Disability Certificate.
  • The person should have minimum of 40% of disability.
  • Medical Certificate should be obtained from the competent Medical authority (ie.) concerned Specialist. In case of multiple disabled person, certificate should be obtained from Medical Board.
  • The person may use the application form allotted for a specific disability and submit the forms at the government hospital or public health centers for obtaining medical certificates.
    • Those applicants with amputation or severe paralysis, and irreversible disabilities are required to get medical certificate from concerned medical practitioner at the government hospital or the primary health centre.
    • Those with multiple disabilities will be certified by a medical board constituted of three doctors, with one of the doctors competent in the field of one of the disabilities the applicant stands incapacitated by.
    • Those affected by cerebral palsy may procure a certificate either from a pediatric, orthopaedic, neurologist, or a psychiatrist.
    • Those afflicted by leprosy will be certified so by a dermatologist, and the hearing impaired by an ENT specialist.
  • Children below 12 years and suffering from mental retardation may be certified of their disability either by a neurologist, pediatric, psychiatrist; and visually challenged will be so certified by an ophthalmologist.

  • Application should be submitted to District Differently Abled Welfare Officer.

Note: Those seeking new national identity cards for the disabled or renewal of existing card are required to register their disabilities certified duly by the aforementioned medical practitioners for the corresponding disabilities as the case may be.

Need for the Document[edit]

The Constitution of India promotes equality, freedom, justice and dignity of all individuals and implicitly mandates an inclusive society for all including persons with disabilities.

According to the provisions of the Persons with Disabilities Act of 1995, it has identified seven categories of disabilities; blindness, low vision, leprosy cured, hearing impairment, loco-motor disability, mental retardation and mental illness.
