Apply for an ID CardEdit
- To apply for an Identification Card in Utah you will need to visit a driver license office.
- Complete an ID Card application, to save you some time you can:
- Fill out and print the application ahead of time online and bring it with you when you go to the office, or
- You can schedule an appointment prior to your visit to avoid waiting in line.
- Have your photo taken.
- Provide proof of birth, full social security number and residence.
- Pay the nonrefundable ID fee of $18.00
- You will receive a temporary photo ID on the same day that you have submit your application along with all the required documents. The completed ID Card will then be mailed to you in approximately three weeks.
RenewalEdit
You may renew your valid regular identification card one time through the mail or internet if it was issued after January 1, 2010. Eligible applicants will be notified by mail of their eligibility and the process to complete in order to renew.

- Receives payment to reinstate your driver license
- Pick up surrendered driver license and/or ID cards

- No age restriction
- Resident of Utah
- Citizen of US or legally present in the US
Note: Effective July 1, 2012 you cannot carry both a Utah driver license and a Utah Identification card.
To those who are no longer capable of driving due to medical reasons or age, you may surrender your Utah Driver License for a Utah State Identification Card free of charge. You must appear in person at a licensing office to take a photo and sign the ID card application.
