- Licence or Permit for Pharmaceutical Business Grade I can be obtained from National Drug Authority secretariat.
- The Applicant must be accompanied by the commitment letter/letter of acceptance by the Pharmacist.
- Fill up the appropriate application form as described by the National Drug Authority.
- The applicant should attach the following documents to support the application.
- A certified copy of the articles and Memorandum of association
- Partnership Deed showing the supervising pharmacist
- Partners of the firm.
- A sketch plan of the premises.
- The certificates of registration of the supervising pharmacist.
- Certificate of registration of the auxiliary pharmacy staff
- The application must be accompanied by a detailed set of plans for the premises, and the application fee.
- After filling the form the applicants shall proceed to pay for licenses to operate pharmacies and drug shops should pay the relevant fees within their area using NDA bank slips.
- Then the applicant shall apply for a certificate of suitability of premises and a Licence to operate a retail pharmacy and wholesale pharmacy.
- The next step shall be the inspection of the proposed pharmacy premises and shall be conducted within 20 days from the date of receipts of the completed application.
- The National Drug Authority shall notify the applicant of this approval and then a Licence to operate a pharmacy shall be issued.
- Collect your Pharmaceutical business Grade I Licence from where your application was submitted within a day from the date the application was received at municipality in case there is no rejection.
- In case the application is rejected the applicant shall be notified in writing.

- Clearance from Ministry of Trade, Industry and Cooperatives for non-nationals.

- Services/business-registration/registering-a-new-company.
- For new business - original certificate of registration (Business name)
- Certificate of incorporation (Company)
- A approval letter from Uganda Registration Services Bureau.
- Original KCCA receipt for the previous year indicating existing business
- Original rent and tenancy agreement from the Land lord for the new business.
- Original trading License for the previous year for already existing business.
- Certificate of suitability of premises
- Licence to operate a retail Pharmacy
- Certificate of Full Name as a Pharmacist
- A dully filled application form for certificate suitability of premises.
- A dully filled application form for the Licence.
- Evidence of payment of prescribed fees.
- Memorandum and articles association ,certificate of incorporation in case of body corporate and partnership deed in case of partnership.
- Certified copies of company form 20 and company resolution appointing the directors in the company one of whom must be a pharmacist.
- Sketch plan of the premises taking into consideration the minimum floor area of wholesale, retail and additional storage area.

- Any person of 18 years carrying out business in Municipality/Division/Town/City/Kampala City Council Authority’s jurisdiction.
- Except those who are exempted under Section 8(2) of the Trade Licensing Act are eligible to obtain Trading license to operate a Pharmacy Grade I.

InstructionsEdit
- All pharmacies in private hospital, medical centers and other institution must apply for license from National Drug Authority.
- A new pharmacy that has been issued with a Licence to operate a pharmacy shall have a clear vision sign post indicating the name and type of out lets.
- A pharmacy may be licensed to operate in other areas. In that case the minimum distance from an existing nearby pharmacy will be 100 meters.
- Dual application for both a wholesaler and retail pharmacy shall not be accepted.

- It is a joint responsibility of the applicant or owner of the pharmacy to ensure that the certificate of the in-charge and auxiliary staff submitted to National Drug Authority are valid and authentic.
