- A. Where a medical practitioner certifies the cause of death
- Step 1 : Obtain Register of Death Certificate (Form 12)
- In the case where the death of an individual has death occurred in a hospital or a medical practitioner has confirmed the cause of death. One should request to be issued the register of death Form 12 by the medical practitioner or hospital.
- The medical practitioner/ hospital complete Form No. 12 Register of Death in triplicate outlining the particulars in relation to the death. These information include
- full names of the deceased
- date of death
- gender
- age of deceased ( years and months)
- occupation
- exact place of death
- normal place of residence
- cause of death (immediate cause)
- other significant information leading to death
- After Form 12 has been filled, you will be given two copies of the certificate to present to the local district registrar’s office.
- Step 1 : Obtain Register of Death Certificate (Form 12)
- Step 2: Obtain Disposal Permit (Burial or Cremation)
- To obtain a disposal permit (burial or cremation) this certificate in duplicate must be taken to the local district registrar’s office.
- Visit your local Division – Town Clerk, Sub County – Sub county Chief office of where the death occurred and present the cause of death certificate (Form 12) to make the application.
- You will be required to produce the National Identification Card or Birth Certificate of the deceased to the registrar’s office.
- Upon submission of Form 12 together with the required documents to the registrar’s office you will be issued with a burial permit.
- Step 2: Obtain Disposal Permit (Burial or Cremation)
- B. Where no medical certificate of cause of death
- Step 1: Obtain Death report
- Visit the nearest police station from where the death occurred and report the death.
- The police officer in charge will conduct you with an interview to establish the cause of death and your relationship with the deceased.
- The police officer wills the issue you with a death report.
- Step 1: Obtain Death report
- Step 2:Obtain Disposal Permit (Burial or Cremation)
- Visit your local Division – Town Clerk, Sub County – Sub county Chief of where the death occurred and present the death report issued by the police.
- The district registrar/town clerk will interview the applicant to determine his/her relationship to the deceased and nature of death.
- You will are required to produce the National Identification Card or Birth Certificate of the deceased to the Town clerk registrar’s office.
- You will then be issued with Form No. 12 Register of Death to fill. The following information are required to fill form No. 12
- Full names of the deceased
- Date of death
- Gender
- Age of deceased (years and months)
- Occupation
- Exact place of death
- Normal place of residence
- Cause of death (apparent cause) (Natural or unnatural)
- Upon submission of Form 12 together with the required documents you will be issued with a burial permit.
- Step 2:Obtain Disposal Permit (Burial or Cremation)
Note:A notification Record for a death that has occurred in a medical facility shall include a document duly signed and stamped by the Hospital- administrator or superintendent.
- Step 3:Obtain Disposal Permit (death occurred in Community)
- For a death that has occurred in the community,a death notification Record shall include a document signed and stamped by the Division – Town Clerk, Sub county – Sub county Chief, and shall have the following particulars of the event:-
- Date of death.
- Time of death.
- Name of Deceased.
- Place of Death.
- Step 3:Obtain Disposal Permit (death occurred in Community)

- A Certificate of Cause of Death.
- A Police Report where Applicable.
- Copy of National ID of Notifier.

EligibilityEdit
- The next of kin or the relative of the deceased present at the time of death of the deceased or dwelling or being in the same area as the deceased
- the occupier of the house in which to the person’s knowledge the death took place or an inmate of the house in which the death took place or any person finding or taking charge of the body of the deceased person or causing the body of the deceased person to be disposed of, in that order
- A medical officer if the death occurred in the hospital
- A officer of the law if the death occurred in a prison or an accident
FeesEdit
- The application is free of charge.
ValidityEdit
- The burial permit is valid until the burial takes place. Then it loses its validity.

Processing TimeEdit
- The burial permit is issued on the day of application.
- Every death shall be registered immediately it occurs and in any case, not more than three months from the date of that death

- Upon registering the death of your loved one, you will be issued with a burial permit by the issuing authority, you can then proceed with the burial arrangements
- The burial permit you required when apply for the death certificate
- Obtain certificate of cause of death from hospital or medical practitioner (Form No. 12) or a death report from the police
- Present the certificate or death report to the Division – Town Clerk, Sub county – Sub county Chief office together with identification documents for the deceased
- Provide relevant information to the Town clerk to obtain burial permit.

Need for the DocumentEdit
- One needs a burial permit in Uganda in order to bury/cremate a body in Uganda.
- There is a No burial without permit in Uganda which states that; No person shall bury, cremate or otherwise dispose of the body of any deceased person the registration of whose death is compulsory, without a permit issued in accordance with Births and Deaths Registration Act.

Other uses of the Document/CertificateEdit
Certificates of cause of death:
- In the case of the death occurring in Uganda of any person who has been attended during his last illness by a medical practitioner, that practitioner shall sign a certificate stating to the best of his knowledge and belief the cause of death.
- Every medical practitioner who refuses or fails without reasonable excuse to comply with this provision shall be guilty of an offence and liable to a fine not exceeding two hundred shillings.
