Uganda - Apply for Change of Name Ownership or Management of a Drug Shop

- The applicant to change name, Ownership or management of drug shop must apply for permission through National Drug Authority.
- Permission to change Name, Ownership or Management of Drug shop can be obtained from National Drug Authority.
- Management of Drug shop is regulated under the National Drug policy and Authority Act.
- All applicants intending to apply for change of Ownership, Name, and Management are strongly advised to familiarize themselves with the relevant laws and regulations and ensure compliance.
- The management, manufacture, importation, prescription and sale of drugs in Uganda are governed by the National Drug Authority.
- Fill up the appropriate application form as described by the National Drug Authority.
- The proposed changes must be supported by suitably qualified personnel such as pharmacists, pharmacy technicians or approved chemists.
- After filling the form proceed to pay the prescribed fee of 100,000Ugshs to the bank under the Name of National Drug Authority.
- Submit the dully filled in form attached with pay slip to the Pharmacist Officer in charge of the name changes/ownership in the area where the premises is located or nearby health district inspector office whose officers shall carryout inspection of your premises.
- NDA shall not be obliged to give permission to change ownership to drug shops opened without following the set procedures and shall not be liable to any claims of resultant financial loss.
- There must be a supervising registered Pharmacist associated with the firm before the application is submitted for change of ownership, name and management as this is a general rule in processing.
- Collect your Permission to Change Name, Ownership or Management of a Drug shop from where your application was submitted within two days from the date the application was received at National Drug Authority in case there is no rejection.
Note:
- It is a joint responsibility of the in- charge and the applicant/owner of the Drug shop to ensure that the certificates of the in-charge submitted to NDA is valid and authentic.
Required DocumentsEdit
All applicants for Drug shop should submit the following not later than 31st January.
- Clearance letter from Ministry of Health
- Previous Approval from National Drug Authority
- Proof of Services/business-registration/registering Drug shop Business.
- Original certificate of registration (Business name)
- Drug certificate verified by the National Drug Authority.
- Certificate of incorporation (Company)
- Proof Letter from Uganda Registration Services Bureau
- Previous Certificate of suitability of premises
- Proof that the drugs enclosed in the application of ownership change are the Essential Drugs Listed in Uganda.
- The pro-forma invoice of Drugs submitted to Ministry of Health (MRU) is also needed.
- Current Certificate of Full Name as Owner of the Drug shop.
- List of items and drugs in the Drug shop.
- GMP certificate (Good Manufacturing Practice (GMP) Certificate this is to certify that the Drug shop meets the international standards of manufacturing medicines.)
- Certificate of Registration (this is a certificate that the drugs in the Drug shop have been registered and it is in use in the country.)
- Regulatory Unit (Ministry of Health) from country.)
- Licence to operate a retail or wholesale Drug shop.
- Certificate of Full Name of Drug shop as a copy of proof of legitimate business units.
- The completed application forms
- Letter of acceptance from the professional in charge plus a copy of his/her relevant Registration/Enrolment Certificate.
- A letter of commitment from the supervising dispenser officer.
- For Retail/Wholesale Drug shop (human): a copy of the certificate of the dispenser or approved registered or enrolled nurse.
- In case of change of Drug shop, the applicant must submit a certified copy of the articles and Memorandum of association or Partnership Deed showing the Supervising Drug shop as one of the Directors or Partners respectively.
- In case of change of ownership, the applicant must submit evidence of the sale such as copies of the sales agreement between the seller and the buyer and the updated /current articles and memorandum of association.
- Two black and white passport photos of the qualified professional in charge.
- A copy of the certificate of the qualified medical/pharmaceutical /veterinary profession in charge.
- Previous Approval from National Drug Authority to Drug shop which had a Licence to operate in the ending year.
- Original certificate of registration (Business name)
- Certificate of incorporation (Company)
- Original rent and tenancy agreement from the Land lord for the new business.

EligibilityEdit
The Applicant to be eligible must meet the following conditions:
- Must have obtain "licensed Drug shop qualification certificate";
- the law, observe professional ethics;
- physical health, can persist in practicing Drug Dispenser jobs;
- Any person of 18 years carrying out a Drug shop business in Uganda except those who are exempted under Section 5(a) of the National Drug Authority policy and Authority Act are eligible for Drug Shop permission to change ownership.
