Apply In-Person:

  1. Application form for registering a death in Tanzania is available in Registration, Insolvency and Trusteeship Agency, Tanzania contact link
  2. Applicant must provide all information in the application form.
  3. Applicant should make sure to carry both photocopies and originals of all documents that are necessary for completing this procedure.
  4. Documents that are needed for completing this procedure is mentioned under “Required Documents” section of this page.
  5. Applicant must attach photocopies of documents (whichever is necessary) along with the application form while submitting it.
  6. Fee for this procedure is listed under “Fee” section of this page for various categories.
  7. If all above-mentioned procedures were followed properly by the applicant, concerned authorities will provide notification to the applicant regarding the availability of the registered document within 1 day.
  8. Applicant can visit the office to collect their document after getting the notification.
  9. If there is no information regarding the document is provided, then, the applicant can visit the office to know status of their application.


Apply Online

  1. Application for registering a death online is available in the following login link
  2. If applicant is an existing user, they can login directly using their “Username” and “Password” and then click on “Login”.
  3. If applicant is a new user, they have to register first by clicking on the “Create Account” button.
  4. On the following page, applicant must provide all details that are requested for new registration and finally click on “Register” for creating the account.
  5. Once the account is created successfully, applicant can login by using their “Username” and “Password”.
  6. After logging in, applicant must click on “Register Death” and the application form for registering a death will be opened.
  7. Applicant must provide all information that are requested in the application form.
  8. Applicant should upload digital copies of documents (whichever is necessary) along with the application form.
  9. Finally, applicant must click on the “Submit” button for submitting the application online.
  10. Fee for this procedure can be paid online by using Mobile Payment Services (M-PESA, TIGO PESA and AIRTEL Money) only in accordance with the information provided to the applicant.
  11. Applicant must specify the district of pickup point to collect the certificate.
  12. If all the above-mentioned procedures were followed properly, the applicant will receive the certificate within 1 day via post.

  • Minutes of family meeting (in case of late registration)
  • Supporting letter from relevant government authorities such as Ward Executive Officer, Village Executive Officer.
  • Passport size photographs of the Applicant

FeesEdit

  • Death that has occurred within 30 days – TZS 3500/-
  • If death is over 30 days and below 10 years – TZS 4000/-
  • If the death is over 10 years – TZS 10,000/-



ValidityEdit

  • The Death Certificate is valid forever.
