Apply In-Person:

  1. To apply for getting an assistive device in person, the applicant has to visit the nearest clinic or community health centre in his/her province.
  2. The contact and address details of the South African Department of Health can be found in the following link: Link
  3. Contact the Department of health and enquire for a nearest clinic or a community health centre in your province. Visit the clinic or community health centre where you will be directed to a rehabilitation service centre or rehabilitation professional.
  4. Go to the relevant rehabilitation service centre and obtain an application form to apply for getting an assistive device as applicable to you.
  5. A professional will assist you in carrying out the prescribed procedure and obtaining the device as per your requirement.
  6. Make sure that you have all the documents requested by the Rehabilitation centre for getting an assistive device.
  7. Provide all the information demanded and submit all the documents required at the concerned department of the Rehabilitation centre.
  8. After all the required processes, the applicant will be served according to his/her requirement.
