Apply Online

  1. To register or obtain license as healthcare practitioners through online, visit the following link: Online portal
  2. If you already have an account, please enter your username and password then click on “LOGIN” button.
  3. If you don’t have an account, click on “REGISTER” button.
  4. Now complete the form with necessary information then click on “Submit” button.
  5. Now you will receive an email with the instructions. In the Email please click on “Activate My Account” button.
  6. In the next page, please enter the new password and click on “Submit” button.
  7. After successful login, click on “Create and Submit Request” option then click on “Apply to Evaluation” option.
  8. Now tick the required checkbox and click on “Save” button then click on “Next Arrow” button which is available on right side bottom.
  9. In the next page, complete the form with necessary personal and professional information then you can upload the required documents mentioned in the required documents section below. Then click on “Save” button to save the entered information and click on “Next Arrow” button which is available on right side bottom.
  10. In the next page, complete the form with necessary educational qualification information then you can upload the required documents mentioned in the required documents section below. Then click on “Save” button to save the entered information and click on “Next Arrow” button which is available on right side bottom.
  11. In the next page, complete the form with necessary work experience and registration information then you can upload the required documents mentioned in the required documents section below. Then click on “Save” button to save the entered information and click on “Next Arrow” button which is available on right side bottom.
  12. In the next page, complete the form with necessary verification report and additional information then you can upload the required documents mentioned in the required documents section below. Then click on “Save” button to save the entered information and click on “Next Arrow” button which is available on right side bottom.
  13. In the next page, pay the appropriate fees through your debit or credit card and click on applicable button to submit your application.
  14. Now the authorities will verify your application and validate the submitted documents.
  15. After verification process, license as healthcare practitioners will be registered.
