STEP 01: Before requesting for a copy of a certified death certificate online, make sure that you have all the necessary requirements with you. To know which document you will need, please check out this link : Required Documents Obtain Death Certificate
STEP 02: To request a death certificate online, click this link - Apply Online.
STEP 03: To start the application process click, “Step 1: Fill out online application form” button (Highlighted with RED box).

STEP 04: In the next page, click this button “Click here to request now” (Highlighted in RED).

STEP 05: Next, the ‘Terms and Condition’ page will be shown. The page contains information about who may request for a certificate, order fulfillment, payment, and cancellation. Once you are done reading the text, click the “I Accept” button (Highlighted in RED).

STEP 06: Complete the Contact and Delivery Information form. After completing the form, click the “Next” button (Highlighted with RED box) to the next step.

STEP 07: A confirmation page will appear. Review all information stated on the form and when you are done, click the “Confirm” button (Highlighted with RED box).

STEP 08: You will be taken to the request page. Click the “Death Certificate” button (Highlighted in RED).

STEP 09: You will need to answer questions 1 and 2 by clicking the “Yes” or “No” buttons. When you are done, click the “Next” button (Highlighted with RED box).

STEP 10: Next, you will be asked to complete a death certificate request form. After completing the form, click the “Save” button (Highlighted in RED).

STEP 11: If you would like to request another type of certificates, you may do so.
You will see on the summary page a list of all certificates that you have requested. At the bottom of the page, you will see the total amount of your transaction (Encircled in PURPLE).
When you are done with your requests, click the “Submit” button (Highlighted in RED)

STEP 12: When you submit a request, you will be issued a Batch Request Number and, for each request, a Request Reference Number. When paying for all requests by batch, specify the Batch Request Number. When paying for a particular request just, specify the Request Reference Number.
STEP 13: Pay for the certificate. To know how you can pay for your certificate click this button, “Step 2 - Pay through accredited payment channels” (Highlighted with RED box). This page contains information on how you can pay for your certificate, how much are the processing fee, delivery, and government taxes.

STEP 14: After your payment is confirmed, all you need to do is wait for your certificate to be delivered.

In the box highlighted in GREEN, enter the Batch Request Number or Request Reference Number of your request in the box below and then click the “Go” button (Encircled in RED).
