• Go to a DMV office
• Complete the form
  • Form DL-54A Application for initial Photo Identification Card
• Submit the necessary documents
  • Social security card
  • Two (2) acceptable forms of address verification documents
  • Pay the necessary fees
  • Let your photo be taken at the Photo Center

Eligibility

• Resident of Pennsylvania
• US Citizen
• At least 10 years old

Fees

$13.50 plus $1.00 ( Organ Donor Awareness Trust Fund ) in check or money order
