Northwest Territories - Obtain Birth Certificate

Apply In-Person:
- To obtain a birth certificate, the applicant will need to visit the Health Service Administration Office, find the address under the “Office Locations and Contacts” section of this page. Here is the contact Link.
- The applicant will need to fill an application form, use the following Application Form link.
- Complete the application form with relevant details.
- The applicant will have to make a fee payment, which is mentioned under the “Fees” section of this page. Using card, cheque or money order payable to the Government of NWT.
- The applicant will need to carry original copies of documents as mentioned under the “Required Documents” section of this page.
- The applicant shall receive their certificate by mail within a few weeks from the submission of the application.

- The applicant shall receive their certificate by mail within a few weeks front the submission of the application.

EligibilityEdit
- The applicant should be applying for their own birth certificate.
- The applicant can be a parent of the person who is the subject of the certificate if that person is under 19.
- The applicant should be a custodial guardian if the parent is incapable, and the subject of the birth certificate is under 19.
- The applicant’s birth should have been registered in Northwest Territories.
FeesEdit
- The applicant will have to pay $22.00 for birth certificates with or without parent information.
- The applicant will have to pay $33.00 for Restricted Photocopy.
ValidityEdit
A birth certificate is valid forever and does not need to be renewed.

The applicant can order 2 types of birth certificates:
- The short-form copy of a birth certificate includes;
- Name of individual
- Date of birth
- Place of birth
- Sex
- Registration date
- Registration number, and
- Date issued.
- The long-form includes;
- Name of individual
- Date of birth
- Place of birth
- Sex
- Registration date
- Registration number
- Date issued, plus
- The names, and the province or country of birth, of the parents.
