Apply by Email or Fax
- To apply for a change of name in Prince Edward Island, the applicant needs to fill out the application form, here is the application.
- Submit the application either by emailing it or by fax.
- Attach all required documents, a list of documents can be found under the “Required Documents” section of this page.

- The application will be forwarded to the relevant authorities.
- Once approved, the applicant will receive their certificate either by mail or another mode, the applicant will be informed.

- A registration statement with a single name issued in the NWT;
- A copy of a valid immigration document with a single name

EligibilityEdit
- The applicant must be at least 19 years old.
- The applicant must be a Canadian citizen, permanent resident or otherwise legally entitled to remain or live in Canada.
- The applicant must be a resident of the Northwest Territories and should have lived in NWT for at least 3 months continuously.
FeesEdit
- Here are the fees that need to be paid:
- Change of Name: $134.00.
- Each additional individual whose name change is included on an adult application: $54.00.
- A duplicate change of name certificate: $27.00
- Use of Common-Law Spouse’s Last Name Statutory Declaration (certified copy): $22.00
- Breakdown of Spousal Relationship Statutory Declaration (certified copy): $22.00

- The document is valid forever and does not need to be renewed.
