Apply By Mail

  1. To Request for Financial Disclosure Report (Court), the applicant has to mail the application to the U.S Court’s Financial Disclosure Office link
  2. The applicant has to download the application form from the following link
  3. The applicant has to enter all the necessary details in the application form (Incomplete application forms are not accepted)
  4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Mail the application form to Financial Disclosure Office, Administrative Office of the United States Court, the address details are mentioned in the “Office Locations & Contacts” section of this page.
  7. The authority will review the application form.
  8. Upon receipt of the Form AO 10A, each judge or employee will be notified that the applicant requested his or her report, then a copy of the requested report will be sent to applicant.
  9. If the applicant is requesting a paper copy rather than an electronic copy the applicant has to pay the prescribed fee as notified by the authority.
  10. The fee to obtain paper copy is mentioned in “Fees” section of this page.
  11. The processing time of this procedure takes up to 7-15 days.

  • To obtain the paper copy of report the applicant has to pay the prescribed fee of $0.20 per page.


Note: The Financial Disclosure Office only accepts checks and money orders for the exact amount provided to the applicant. The applicant’s check or money order should be made payable to the Administrative Office of the United States Courts. Paper copies of a report will not be mailed until payment has been received.
