1.Death in a health facility

  • When death occurs in a health institution, a hospital death certificate is issued by the administration of the institution. Here upon death, health personnel probably present at death obtains the hospital death certificate form and fills in the information about the deceased and cause of death;
  • Then thereafter take the form is taken to a nearby registration office (civil registration conservatories and district office or the administrative posts )for death to be recorded;
  • At the registration office details of the deceased person are recorded in the death register and a death certificate is issued;

2.Death not in a health facility

  • You should report death to the local authority immediately after death has occurred;
  • The authority writes a letter declaring death of the deceased person;
  • Then take the letter to a civil registration office (civil registration conservatories and district office or the administrative posts) where details of the deceased are issued and thereafter a death certificate is issued;

3.Death abroad

  • When death occurs out of Mozambique, look for a Mozambican diplomatic mission in the country. where death has occurred or in a neighbouring country (if there is no Mozambican diplomatic mission in the country where death occurred);
  • Inform the authority of the identified Mozambican diplomatic mission about the death of the deceased;
  • The occurrence shall be recorded and a death certificate is issued

1.An applicant reporting death must have detailed information about the deceased;

2.If possible death should be reported in company of a witness;

3.Death should be reported as soon as possible;

4.Death can be registered by any person present at death;

3.Date and place of birth of the deceased person

4.Sex, age and marital of the deceased person at the time of death

5.Time and date of death of the deceased

6.Place of death

7.Cause of death

8.Place of residence at death

9.Place of burial

10.Details of dependants left behind (Spouse and children)

11.Details of parents (names and address, nationality of both parents, occupation of both parents and dates of Birth of both parents)

12.Details of witnesses present at death (the attending physician, midwife, birth attendant or other witnesses)



Need for the DocumentEdit

Death registration is the official and permanent recording of a deceased person by the State. Death registration in Mozambique is administered by the Ministry of Justice through the National Directorate of the Registries and Notary (Direco Nacional de Registos e Notariado, DNRN). The Directorate ensures that all deaths are registered through civil registration conservatories, hospital posts and district office or the administrative posts.
