Apply by Mail
- To Request Transcript Order (Court), the applicant has to approach the United States Court link
- NOTE: Search by address, city, state or ZIP Code and click the “Location” button.
- The applicant has to download the application form from the following link
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the court mailing address.
- The mailing address is mentioned in the “Office Locations & Contacts” section of this page.
- The court will notify you of the amount of the required deposit fee which may be mailed or delivered to the court. Upon receipt of the deposit, the court will process the order.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- The court will notify the applicant when the transcript is completed.
- Once the process gets completed, the respective authority will send the transcript order via mail or email.
- The entire process is complete within 3 weeks or it will intimate by the respective officer.
- NOTE: For each of transcript, the applicant has to submit separate forms.
