Apply In-person

  1. To obtain a ID card for disabled, applicant has to contact the respective “Superintendent/In-Charge of Medical Colleges & Hospitals and Civil Hospitals/CHCs/PHCs/SHCs/SDs.” for their area with the documents.
  2. Link for contact: Online link please click on name of the district on the map to reach the district portal. In the given page, select the menu “Directory > public utilities > Hospitals” for respective contact details or
  3. Please use the link : link and select the state name from the drop down list similarly the district name and hit go to get contact details.
  4. Apply in writing or use the application form as per the following link: Form link
  5. Please submit the completed application along with required documents to the authority. After validation and checking of the document is done, further process will be carried out.
  6. Applicant has to follow the guidelines and instruction to complete the department process for considering the application to grant a card.
  7. Finally Disability Card will be provided by the respective “District Social Welfare Office”.
Note: The Director General of Health Services, Ministry of Health and Family Welfare will be the final authority to issue the ID card based on the documents.

Following listed information provides you details on who needs to be approached for getting appropriate certificate.

  1. Children below 12 years and suffering from mental retardation may be certified of their disability either by a neurologist, paediatric, psychiatrist and visually challenged will be so certified by an ophthalmologist.
  2. Applicants may use the application form allotted for a specific disability and submit the forms at the government hospital or public health centres.
  3. Those applicants with amputation or severe paralysis, and irreversible disabilities are required to get medical certificate from concerned medical practitioner at the government hospital or the primary health centre.
  4. Those with multiple disabilities will be certified by a medical board constituted of three doctors, with one of the doctors competent in the field of one of the disabilities the applicant stands incapacitated by.
  5. Those affected by cerebral palsy may produce a certificate either from a paediatric, orthopaedic, neurologist, or a psychiatrist.
  6. Those afflicted by leprosy will be certified so by a dermatologist, and the hearing impaired by an ENT specialist.

  2. click on citizen login or New user? register here…create new user account. Once it created, log in using username and password.
  3. Window will appear
  4. Then search for Social Justice & Special Assistance Department and click on it. Then click on Issue of Disability Identity Card To Person With Disability.
  5. Then fill necessary information required like Name of husband, District name, Taluka name.
  6. A window will appear this will be an application form. Fill it carefully and click on submit
  7. Then window will appear which shall ask you to upload the passport size photograph and all required document.
  8. Click on upload documents
  9. Post click on submit the window will appear which will ask you for payment then select proper payment option, make payment and submit
  10. Then after 30 days you can download soft copy of senior citizen certificate



Apply Online - Maharashtra Apply Identity Card for Differently Abled Online with Images

  1. To apply online for disability card online, please follow the link given. Link for contact: online portal
  2. Please select “Apply for Disability Certificate & UDID Card” option provided on the right side part of the screen.
  3. A new page to enter / select details will prompt user to fill in the required details.
  4. User can select language of choice using the “Choose regional language” option and click select your language from the drop down menu.
  5. There will be four parts to process. First part, personal details, followed by Disability detail, employment detail and Identity detail. Please fill in the required information for each part and hit “next” tab or the required option to go for the next part till the employment details part.
  6. At the end of the screen user has to click the “terms agree..” option and then hit precede button to preview his application.
  7. If the displayed details are Ok hit Confirm application button to complete the online application.
  8. Applicant will get the enrolment / UDID / request number. Please keep it safe for future reference.
  9. After the processing and verification of the details provided as per the norms. Applicant will get notification about the process. If he is granted with a disability card, then one can visit the online portal and download the e-UDID card.



Note: In case of multiple disabilities, the application will be forwarded to the District Medical Board for examination and issue of certificate.

Apply through Mobile APP:

  1. To apply via app, the applicant shall download the app from the given link: Link
  2. Select the preferred language
  3. A list of options will be provided from which the applicant must choose the concerned department
  4. Now a list of services will be displayed from which the applicant must select the required service
  5. The applicant must also provide the nature of service verification and click on “Apply” to proceed to verification
  6. Once verified, the applicant will be informed about the further process.



Apply Through Centres:

  1. To apply for Identity Card for Differently Abled through centres, the applicant has to visit the respective “CSC” centre/sewa kendra. Please tap on the following link to locate the centre according to the respective district: link.
  2. Make sure to have the documents that are listed under the “Required Documents” section of this page.
  3. An application form can be obtained through the kiosk operator upon visiting the centre.
  4. The applicant has to fill in all the necessary information in the application form.
  5. Submit the application form with “Required Documents” to the office. It is advised to take the documents in original to the kiosk for scanning purposes.
  6. The operator will process the application online.
  7. On successful submission, the applicant will receive an application number as acknowledgement. The applicant will also get a transaction number as a text message on their registered mobile number.
  8. Pay the fee as given in the “Fees” section of this page and collect the receipt.
  9. An ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
  10. Upon the satisfaction of the authorities, the permission will be granted according to the application.

ValidityEdit

  • This card is valid for a period of five years from the date of issue.
  • On expiry the card should be surrendered to the issuing authority and an application for issuing a new card should be submitted if required.
  • In case the card is lost, new card will be issued with the payment of a penalty.

Need for the DocumentEdit

  • The Constitution of India promotes equality, freedom, justice and dignity of all individuals and implicitly mandates an inclusive society for all including persons with disabilities.
  • According to the provisions of the Persons with Disabilities Act of 1995, it has identified seven categories of disabilities: blindness, low vision, leprosy cured, hearing impairment, loco-motor disability, mental retardation and mental illness.
  • Persons with Disabilities are provided with financial aid from the state government.
