Obtain forms IX or download from KMPDC website; fill and submit to the Board.

Attach the following documents:-

  • Company or Business Registration Certificate
  • Certified copies of Professional Certificates of all Health Professionals working therein including licenses from their respective Regulatory Authorities.
  • Building Architectural Plans for hospitals and nursing homes.
  • Hospitals and Nursing Homes must have a mortuary or a cold room for dead bodies.

All in-patient facilities must have a clearly defined method of waste disposal and management systems in place including incinerators, placenta pits etc.

The facility should be inspected by the County Director of Health or County Health Management Team or Joint Board and Council Inspection Team.

It may be a requirement from other Health Regulatory Authorities to obtain their licenses for other facilities: e.g. Laboratory, Pharmacy, Radiology and X-ray Services etc.

The Proprietor/Administrator/Director must ensure that: -

  • All payments are paid in full as per the category.
  • Licenses are up to date.
  • Health Professionals working therein are registered and licensed by their respective Boards/Councils.

  • Evidence of employment/job offer in a recognized Institution. Private practice is not allowed on licence to render

InstructionsEdit

The application form must be returned to the Medical Practitioners and Dentists Board within a period not exceeding three months from the date of issue. Applications which are not returned within the stipulated period shall be time barred.

  • Contact details of the proposed institution
  • Whether Hospital, Nursing Home, Maternity Home, Health Centre, Dispensary, Laboratory, etc.
  • Location of the institution
  • Full names and address of the applicant
  • If applicant is a director and/or administrator of the institution
  • Nationality of the applicant
  • Place and date of birth
  • Kenya national identity card no
  • Passport no. (if applicable)
  • Email address
  • Work permit no. (if applicable)
  • Full names of Directors of the institution including the following: Nationalities, Passport
  • Numbers, Work Permit Numbers, Email Address, Kenya National Identity Card Numbers, etc
  • Full names and registration number of the medical or dental practitioner who shall be in-charge of the patient health care at the proposed institution
  • Full details of professional qualifications of the person named at paragraph above
  • Full names and professional qualifications of any other person(s), identified by your institution to undertake patient health care at the institution (e.g., Clinical Officers, Nurses, Laboratory Technicians, X-ray Staff, Doctors, Technicians, Pharmaceutical Technicians, etc.).



Need for the DocumentEdit

Doctors and dentists aspiring to practice in Kenya must comply with the statutory requirements for registration and licensing. Failure to obtain the necessary documentations constitutes an offence under section 22 of the Medical Practitioners and Dentists Act. Detailed information on documentation can be accessed via the Boards website and from the Boards offices.


Information which might helpEdit

It shall be the duty of every practitioner to practise in medical clinics, dental clinics and health facilities approved by the Board.

Every institution or practitioner shall be required to employ duly licensed and or retained practitioners, as the case may be.

Home visits will require the practitioner to be duly registered and licensed to indulge in private practice as prescribed by law.

Mobile clinics shall be required to be registered and licenced by the Board.

There shall be notification at least two (2) weeks of intent to hold any medical and/or dental camp to the Board and to the PDMS/PDPHS.
