Procedure

Apply in-Person

1. To apply for the employment injury, benefit medical certificate, you should submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). please view the NIS contact details in the “Offices and Contact” section below
2. Obtain the application form from the NIS office in person or download the form from the MLSS website through this link:Link (choose “Employment Injury Benefit Medical Certificate” and download the form
3. Complete the form and take it to a certified doctor to complete the doctor’s certification section
4. Then prepare the required documents and attach them to the application form
5. Once your application form and all supporting information and documents have been completed, signed, stamped, and certified where required, go to the NIS office and submit the application to the NIS official at the service desk
6. Then the NIS will verify your application and confirm if the application meets the required standards
7. After assessment of the application and the NIS determines that you are not fit to work due to your injury or injuries they will certify and approve your benefit application
