Procedure

Apply In-Person

1. To inform the National Insurance Scheme (NIS) of the name change of an employer, you have to submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). Please view the NIS contact details in the “Office Locations & Contacts” section below
2. Obtain the application form from the NIS office in person or download the form from the MLSS website through this link: Link (choose “Employer's Name Change” and download the form)
3. Complete the application form correctly and enter the dates following the format provided in the application form
4. Prepare the required supporting documents and attach them to the application form
5. Once your application form and all supporting information and documents have been completed, signed, stamped, and certified where required, go to the NIS office and submit the application to the NIS official at the service desk
6. The NIS will verify your application and confirm if the application meets the required standards
7. Once everything has been assessed and the application is successful, the NIS will process the name change application and update their data
