Jamaica - Add Fathers Particulars on Birth Record

Procedure[edit]

Apply In-Person

  1. You need to obtain the application form for addition of fathers particulars from the Registrar General Department (RGD) office in your area or download it directly form the RDG website via this link: Link under “documents and forms” or download it directly from then link provided in “Documents to Use” section below.
  2. Both you and your partner, complete the application form in the presence of a Justice of the Peace (JP), Attorney of law, School Principal, Clerk of the courts, or a Notary Public if either of the one of parents are in abroad and have copies of the required documents mentioned in the “Required Documents” section below certified by a Notary Public or the JP office
  3. Then attach all the required supporting documents to the application form and visit the RDG office in your region to submit the application at the RDG’s services section.
  4. An RGD official will verify your application and if it is correct, proceed and pay the required application fee: note that you can opt for the express service at higher fee than the normal service.
  5. Upon payment of the fee, the application will be forwarded to the responsible authority for processing and you will be issued an acknowledgment receipt with details of your application.
  6. The application file will be processed and you will be issued the new birth certificate with the child’s father particulars add to it.



Application By a Proxy

  • If both you and your partner are unable to submit the application form in person, you can choose a third party who will submit the application on your behalf provided, they have a letter of authorization form you the parents. Note that the letter must state the name, address and phone number of the owner of the account (i.e. the child, parent, husband or wife) and it must also show approval for the 3rd party to conduct the transaction for you and it must be certified by a Notary or a Justice of the Peace (JP)

Eligibility[edit]

  • Jamaican citizens and residents whose child birth certificate was issued in Jamaica in general the declarant/applicant should be a family member who is at least 10 years older (parents, grandparents, aunt, uncle, sister/brother)

Fees[edit]

  • Normal services within 6 weeks: $1500
  • Express services between 7 to 10 working days: $3500


Validity[edit]

  • Birth Certificate is Valid Forever

Instructions[edit]

  • Declarants must provide identification and when completing the form, all signatures should be the same as what is on the ID.
  • If errors were committed while completing the form it must be crossed out and initialed by the same signing Notary Public or J.P.
  • The date when the form was signed by the Notary Public or J.P. must be the same as the date signed by the Declarants.
  • Before submitting the form, make sure that you provide all required information. Incomplete application will not be processed.
  • Interview sessions for the signing Declarant are Mondays to Fridays, 8:30a.m 2:00p.m.
