In Person

You may go to any county health department in the State of Idaho to obtain a certified copy of an Idaho certificate.

Provide all necessary information and pay the certificate fee. You may pay for the certificate in cash, check or money order payable to Idaho Vital Records.

Most Idaho certificates can be issued while you wait.


By Mail

  1. Fill out a Certificate Request Form or write a letter containing all the necessary information.
  2. Include a photocopy of your acceptable identification and a check or money order for the certificate fee made payable to Idaho Vital Records.
  3. Send your request to:

Standard Application - Application process will take five to seven working days after application is received and verified in our office.

Rush Application - Application process will take a few working days after application is received and verified in our office. Please add an additional $5.00 for rush applications and write RUSH on the outside of your envelope.


Applying Online - VitalCheck

Go online and simply follow the steps to place an order. An additional fee is charged by VitalChek for using this service and all major credit cards are accepted.


By Fax

Faxed requests are handled by VitalChek.

Write a letter containing the following information:

  • Type of certificate (birth, death, marriage, divorce)
  • Full name on the certificate
  • Date of event
  • City of event
  • If ordering a birth certificate include the father's full name and mother's full maiden name
  • Your relation to the person whose certificate you are ordering; only immediate family, legal guardian or legal representative may order certificates - proof of relation may be required
  • Purpose for the certificate
  • Your return address (must be a street address for express delivery)
  • You will need a photocopy of your acceptable identification, your credit card number and expiration date for the VitalChek certificate fee
  • Daytime phone number where you can be reached during business hours
  • Delivery method: must be regular mail or express service

Online orders that notate and pay for Express Service will be processed within a few working days after the request is received and verified.

Online orders sent through U.S. Postal Services will be sent in five to seven working days after the request is received and verified.

EligibilityEdit

  • By Idaho state law, Idaho Death certificates are confidential records and can only be obtained by the following authorized individuals:
    • Mother/Father (or Legal Guardian) of Person Named on the Idaho Death Certificate
    • Husband/Wife of Person Named on the Idaho Death Certificate
    • Grandparents of individual named on the Idaho Death certificate
    • Son/Daughter of Person Named on the Idaho Death Certificate
    • Grandchildren of individual named on the Idaho Death certificate
    • Sister/Brother of Person Named on the Idaho Death Certificate
    • Legal Representative of an Authorized Person of Person Named on the Idaho Death Certificate
    • Individuals who provide documentation showing a Idaho Death certificate is needed for their property rights
  • Access to copies of Idaho death certificates is restricted to the decedent's immediate family. If you are not a member of the decedent's immediate family, you will need to fax proof of entitlement to 866.559.9629 upon completion of your online order.
  • Anyone can obtain an Idaho Death Certificate, 50 years or later from the date of the Death for genealogy or other reasons.

  • Each additional copy of birth, stillbirth, marriage or divorce ordered at the same time: $13.00
  • Each additional copy of death certificate ordered at the same time: $14.00


Searches for Vital Records (Per Event):

  • Searches for birth, stillbirth, marriage or divorces: $14.00
  • Searches for death: $14.00

Notes:

  • You will need to add $5.00 for each order requiring special handling when requesting a certified photocopy of the original record, or requesting expedited processing. If you would like to RUSH your order, you will need to include a one-time charge of $5.00 (per order) and write RUSH on the outside of your envelope.
  • There is no shipping charge for regular mail. If express mail is desired, you may express mail your request and include a prepaid express mail envelope back to yourself. Orders cannot be sent, C.O.D.
  • If you are sending your request for certificates by fax, there will be an additional $10.50 in special handling fees.

InstructionsEdit

  • The Idaho Bureau of Vital Records (Idaho Vital Records) issues certified copies of Idaho death certificates for events that occurred in the State of Idaho.
  • Idaho Vital Records Unit Bureau has some records since July 1911. For records from 1907 to 1911, write to County Recorder in county where death occurred.
  • You may order copies of Idaho vital records in person, by Mail, online (VitalChek on an expedited basis), and by fax.
  • Fees may be paid by cash (except for applications made by postal mail), money order, certified check, or cashier's check - Make checks or money orders payable to Idaho Vital Records.
  • You can request certificates through Standard Application and Rush Application
  • Online orders are processed through the VitalChek. It is an independent company that we have partnered with to provide credit card and online orders.
  • Fax request are handled by VitalChek
  • Any request requiring an amendment or legal action may take two to three weeks after the request is received and payment is accepted.
  • Online orders sent through U.S. Postal Services will be sent in five to seven working days after the request is received and verified in our office.
