ProcedureEdit

  1. Obtain a duplicate license via mail, send Letter Requesting for a Duplicate of your Hawaii Driver License.
  2. Include Your Name, Social Security Number or Hawaii Driver License Number, Date of Birth, Email address (optional), reason for the request, Signature
  3. Do not forget to include your payment, $6.00 (certified check or money order payable to City & County of Honolulu).
  4. Mail your request to:

You may also fax your request to (808) 832-2904


Notes:

  • If there is no compliance requirement pending in any jurisdiction that prevents you from getting a license, your duplicate license will be mailed to the address on file.
  • If your image is found in the driver database, both the photo and the signature will be utilized.

You may apply for a duplicate license if you are changing your name or address; do not have your license in your possession (lost/stolen) or your license is damaged.

Driver License, Provisional License and Instruction Permit and Related Fees

Road Test : $8.00

Written Test: $2.00

Oral Test (Available at City Square and Wahiawa): $10.00


Instruction Permit : $5.00

  • Renewal: $5.00
  • Duplicate (to replace lost, stolen or damaged): $6.00


Provisional License:

  • For those aged 16 thru 18: $5.00 per year or fraction thereof
  • Duplicate (to replace lost, stolen or damaged): $6.00


Driver License :

  • 2-Years (age 72 and older) : $10.00
  • 4-Years (ages 17 thru 24) : $20.00
  • 8-Years (ages 25 thru 71) : $40.00
  • Duplicate (to replace lost, stolen or damaged) : $6.00
  • Reactivation (renewing after 90-day grace period) : $5.00 per 30-day period
  • Reinstatement: $20.00
  • Non-US Citizen required USCIS verification : $1.00


Forms of payment that are accepted at Driver License Locations:

Cash, Visa, MasterCard, Discover or
Check made payable to City and County of Honolulu.
