Procedure

Apply In-Person:

1. Applicant shall approach their respective civic agencies (municipal/corporation/local body) of their respective area from who applicant has already obtained license for pet dogs.
2. Go to the respective office. The contact details are given below in the “Office Location”.
3. Meet the respective authority at the office. Applicant shall get the advice from the authority to be followed in order to register the change of address.
4. Collect the applicable application form as per advice.
5. Submit the completed application form along with required documents, as mentioned in the “Required Documents” section, primarily the proof evidencing the change of address.
6. Authorities will check the submitted application and announce the fees / charges. Please pay and get the receipt.
7. This application will be processed further. Follow up with the authorities for notification regarding the status of the application.

Need for the Document
