Apply In-Person:

  1. To Apply for Death license in Ghana, the applicant has to approach the ADA East District Assembly office or Birth and Death Registry Headquarters office.
  2. Address and contact details can be found in the following Contact link
  3. Applicants can get their application form from the relevant department.
  4. Applicant has to enter all the information in the application form.
  5. Applicant must make sure to carry all the necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
  6. Applicant should attach photocopies of documents (whichever is necessary) along with the applicant from while submitting it at the office.
  7. Applicant will be required to pay the appropriate inspection fees and submit the payment receipt. The amount of fees to be paid can be found under the "Fees" section of this page.
  8. If all the above mentioned procedures were followed properly by the applicant, the application form will be viewed by concerned authorities and the applicant will be notified regarding their license.
  9. This application will be processed and information will be verified by the AEDA office.
  10. If the verification is successful, notification will be sent to the applicant.
  11. After seeing this notification, the applicant must go to the respective office and the applicant can get their license.
  12. Applicant’s death license will be ready after 14 working days.

EligibilityEdit

  • Any citizen and foreigner who is death in the Ghanaian city and this can be processed by a relative or next of kin, an inmate of a house where death occurred, any person who finds or takes charge of the body.

  • Online application fee- GHS 100.00

Other uses of the Document/CertificateEdit

The death certificate is important for three basic reasons:

  • Legal Reasons: The death certificate is a permanent legal record of the fact of death. Oklahoma law stipulates that a death certificate is to be filed (63 OS 1-317(a)). Therefore, it is a legal requirement. It provides important information about: the decedent, the cause of death, and final disposition. This information is used in the application for insurance benefits, settlement of pension claims, and transfer of title of real and personal property. The certificate is prima facie evidence of the fact of death and, therefore, can be introduced in court as evidence when a question about the death arises.
  • Personal Reasons: The death certificate in many cases provides family members with closure, peace of mind, and documentation of the cause of death. It also provides peace of mind by facilitating efficient processing of needed benefits as those described above.
  • Vital Statistics Reasons: The death certificate is the source for state and national mortality statistics. It is needed for a variety of medical and health-related research efforts. It is used to determine which medical conditions receive research and development funding, to set public health goals and policies, and to measure health status at local, state, national, and international levels. This data is valuable as a research tool and by influencing research funding.
