Procedure

How to Apply:

1. You should visit the Caisse Nationale de Sécurité Sociale (CNSS) office near you and express your interest at the service desk.
2. Application form can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download :(Application form)
3. Submit the completed duly filed application form along with the supporting documents (listed under “Required Documents” section) for verification and processing to the attending official at the CNSS.
4. Information obtained from the application form will be entered into the CNSS data system and You will be registry and issued with a registration membership card

Required Documents

• 1 legalized copy of birth certificate or supplementary judgment (for nationals)
• 1 Individual civil status sheet (for foreigners)
• 2 passport photos (on white background)
• Legalized copy of the nationality certificate for naturalized persons (to be completed with the birth certificate)
• Any other document that may be requested by the CNSS official

Employee/ workers either working in government or private sector are required of registration with the CNSS immediately and obtain a registration number
