Procedure[edit]

When the birth occurs in a hospital:

The midwife shall report to the registrar in residence authority (the municipality in Jutland or parish in the rest of the country).

When parents are married to each other, the paternity so registered.

Are not married to each other and want to get together to care and take responsibility for your children, you can declare it using the form "Caring and responsibility statement".

When authorities receive care and responsibility statement within 14 days after birth, is paternity established. I automatically get joint custody of your child.

You can report your child's birth and paternity electronically with digital signature.

You can also obtain a form of midwife or even print it out.

Fill out the form and send it to the parish where you live.

At home deliveries:

Required Documents[edit]

The mother's original birth or name certificate must be submitted along with the birth notification. If the parents are married, the father's birth or name certificate and the parents' marriage certificate must be submitted as well. The mother is under an obligation to state who the child's father is. With regard to inheritance and alimony the father's identity should be stated on the birth notification.

Instructions[edit]

Following a birth, the midwife fills in the forms to register the birth and sends them to the Registrar of the State Church, who issues a birth certificate. These forms have to be handed in at the local State Church office (even if the family are affiliated to another religion or do not attend church) in the area of residence within 14 days of the birth.

  • Names, occupation and residence of parents, godmother, sponsors

Need for the Document[edit]

A child's birth must be reported. Usually, the midwife makes the notification, but if the birth happened without a midwife, parents must report the birth. You can report your child's birth through the form "Notification of Birth" within 14 days after birth.

Other uses of the Document/Certificate[edit]

It is convenient for a society to know who lives and is born in the country. A birth must be registered within 14 days after it has occurred. The child is thus recorded in the CPR.

When your child is born, the authorities must be notified of the birth within 14 days so that the child can be given a Social Security number and a birth certificate

If the father of a child born out of wedlock, want to acknowledge paternity, he may, in connection with the registration of the birth of a written undertaking (a so-called care and responsibility statement) thereof. The form can be found and completed in personregistrering.dk

The form must be submitted together with the birth registration. This gives parents include joint custody of the child.
