Procedure

Apply In-Person

1. To report malpractice in different government welfare scheme, please visit the Directorate of Central Government Health Scheme. The address and contact details of the office can be found in the following Link.
2. Visit the Office and get the application form from the relevant department or the applicant can also write the complaint on a plain A4 Sheet to report malpractice in different government Welfare scheme.
3. Please fill the application form with essential information and details and it should be attested with the applicant’s signature.
4. Please attach all the documents that are listed in “Required Documents” section of this page.
5. Then, it should be submitted to the relevant authority. Also pay the required fees for the application process at the office as directed by the officials. Please collect the acknowledgement slip for the paid fee. Keep it safe for future reference.
6. The application and attached documents will be sent for verification to check, if there is any discrepancy.
7. After all the processes are completed, concerned officials will look into the problem and forward it to the Director General.
8. The grievances at Wellness Centre level can be sorted out in the "Advisory Committee" meetings held on every 2nd Saturday of the month. Grievances related to medical reimbursement claims can be sorted out on Claims Day held in the O/O Additional Directors Zone/city on every 3rd Thursday of the month, with prior intimation and Claims Adalat  held every six months.
9. The Director General will analyse the problem andproceed with solving your grievance by taking necessary measures.
