Procedure

1. Fill up the application form.
2. Submit the form along with all other required documents to the Department of Civil Registration and Census Headquarters.
3. The applicant has the option of collecting the corrected CID/SRP card from DCRCO/ Thromde/ Department of Civil Registration and Census HQ offices.

Required Documents

• Completed application form
• For Civil servants: copy of service record certified by the RCSC along with class X, XII and degree certificates or highest academic certificate obtained (original certificates must be produced for authentication)
• For those serving in the Armed forces: copy of service record certified by record officer along with class X, XII and degree certificates or highest academic certificate obtained (original certificates must be produced for authentication)
• For those serving in the judiciary, autonomous agencies and corporations:copy of service record certified by personnel/HR officer along with class X, XII and degree certificates or highest academic certificate obtained (original certificates must be produced for authentication)
• For monks and nuns:certification byZhungDratshang /Uzin/Head of Institution
• For students:copy of academic certificate or highest academic certificate obtained (original certificates must be produced for authentication)
• For others:certification by Thromde CRCO/Tshogpa&Gup

Individuals who are registered with Department of Civil Registration and Census / MOHCA.

Fees

• Application is free of charge

Processing Time

Applications for name change/correction of date of birth are usually processed for one day from the time the application was submitted.

Corrected cards will be delivered to the applicant within 7 days.

Applications for Change of Name / Correction of Date of Birth can only submitted at the Department of Civil Registration and Census HQ only.
