Following are the ways through which you can apply for family member certificate in Andaman and Nicobar.

Apply In-Person:

  1. To apply for the family member certificate approach the Tehsildar officer who is regulated under the Department of District Collectorate.
  2. Get the application form for applying family member certificate from the tehsildar office.
  3. Make sure that you have all the proper documents mentioned in the “Required documents” section below.
  4. Submit the form to the District Collectorate office along with the required documents. More details about the District Collectorate office location is provided in the following Office Location
  5. Refer the District Collectorate Office location mentioned in the “Office location & contacts” section below.
  6. After the application form is processed, an enquiry officer will make a thorough verification regarding your proof which you have submitted along with the form.
  7. Collect the acknowledgment slip after submitting the form.
  8. Use acknowledgment slip to obtain family member certificate.

Apply Online

  1. To obtain a family member certificate in Andaman and Nicobar Islands online, the applicant shall follow the procedure given here: Link.
  2. Click on the link to visit the citizen login portal.
  3. Enter the valid Email id and password if already a user, otherwise, create a new account by clicking “Create account” in the citizen login.
  4. Now, click on the “Useful service” which is displayed on the home page of the portal.
  5. Select the application for a family member certificate from the given list of options.
  6. The application form for the family member certificate will open up on the next screen.
  7. Fill out the application with the required details and upload the prescribed documents mentioned in the "Required Documents” section of this page.
  8. Finally, click on the “Submit” button after completing the details in the application form.
  9. Upon submission, the applicant will receive an acknowledgement slip as the confirmation of submission.
  10. After verification, the applicant can obtain a family member certificate from the concerned authority of the District Collectorate Department with the help of an acknowledgement slip.

Required InformationEdit

The following information is required to complete the application form.

  • Full Name of the applicant
  • Other family members names (who are alive)
  • Applicants Date of birth
  • Relationship to the applicant
  • Every family members date of birth
  • Every applicants occupation
  • Passport Size photographs of the applicant
  • Gazetted officer should duly attest the application form
  • Gazetted officer should duly sign on the bond paper with the proper proforma
  • Xerox copy of the family head death certificate
  • Xerox copy of the completed application
