How to Apply

Bed-Side Registration of Birth
- Bed-side registration happen in the hospital where the birth has occurred, a Registrars stationed at the hospital obtains information on mothers who delivered babies from the hospital’s delivery book and by physical observation. Note that this information is then used to account for and locate all mothers who have delivered and whose babies must be registered by the registrar
- The Registration officer or assistant will then visit the Maternity or Labour ward upon delivery of the child and interviews each mother and the Information provided by the mother is compared with information recorded in the hospital’s delivery book.
- Upon verifying the information provided, the registration officer will then record the particulars of the birth on the registration form, including these details: place and date of birth, physician or nurse in attendance of delivery of the child as well as mother’s particulars and the new born parents contact telephone numbers. Then the mother or father of the new born child will also sign as the informant
- If parents of the new born child are unmarried, and if the father with the mother’s approval presents himself at the point of registration, he is asked to sign the birth form along with the mother for the A&B registration and also, within six weeks the mother and father can complete a Declaration of Paternity form to add the father’s particulars. You will be required to present your identification documents and the registrations are usually performed during visiting hours when the fathers are allowed in the ward
- Then the Registration Officer will inform you the parents of the new born that if you name your child in hospital that you will be issued one free birth certificate
- If you decide to name the child while in the hospital, you shall give the Registration Officer the name of the child as well as the correct spelling and the name is registered.
- You will then be required to pay a registration fee, after which the mother is issued with a Certificate of Registry of Birth (C-reg.), signifying proof of registration. This certificate also contains the birth entry number.
- Now visit any of the RGD office with the C-reg to apply for one free copy of the child’s birth certificate two months after the registration. Note that you can apply for addition copies at a fee.
- If the child was not named in the hospital or the name is changed after registration in the hospital, the Registration Officer will issue a blank Certificate of Naming form to you, the parent/s and instructs you to complete the form and visit the Registration Centre within six weeks to name the child.
- Complete the Certificate of Naming and submit it to the Registration Centre within six weeks to one year to name the child and the name will be entered on line 26 of the registration form
- You will then be required to pay a registration fee after which the mother is issued with a Certificate of Registry of Birth (C-reg.), signifying proof of registration. This certificate also contains the birth entry number
- Go to any of the RGD office to apply for the birth certificate at fee mentioned in the “Fees” section below
Birth outside the Hospital or in a Birthing Centre Registration
- Once the birth has occurred, an informant will be required to visit the Local District Registrar (LDR) to register the birth. The informant can a parent then new born child, person present at birth, an occupier of the house in which the birth occurred or any other adult who is aware of the birth of the child. Note that the informant must visit the LDR’s office within six months, or at least within one year after birth. Applications after one year of the birth will lead to a late registration application.
- If you the parents of the child are not married, both of you must visit the LDR for the father’s particulars to be added at time of registration, and complete the A and B registration.
- Then the birth registration form will be completed by the LDR officer and you will be required to sign the form in the relevant section.
- Upon completion of the registration form, you will be required to pay the registration fee after which you will be issued a Certificate of Registry of Birth (C-reg.), signifying proof of registration. This certificate also contains the birth entry number.
- And then the original registration is sent to the Head Office of the Registrar General's Department (RGD) within six weeks of the registration
- Visit any of the RGD office to obtain the birth certificate at a fee

- Once the birth has occurred, visit the Registrar General's Department (RGD) office in your perish to register the birth and obtain the birth entry number
- You can also obtain a birth entry number through online via the RGD online application portal through this link:Link fill the online form and submit to obtain the birth entry number
- Once you have obtained the birth entry number, visit the RGD online portal to submit your application for the birth certificate through this link:Link
- Read the provide information and proceed to the “Birth Certificate” application form if you have the birth entry/registration number, click on “APPLY NOW” to fill the online application form
- Complete the form accurately providing all the required information to avoid unnecessary delays on your application and then submit it for processing
- Once submitted you will be required to pay the necessary application fee according to the number of copies that you have applied for
- Once the payment is complete, the RGD will process your application and issued you the birth certificate
