Procedure

Apply In-Person

1. To change or update your name in your Jamaican driver’s license due to marriage, a deed poll, adoption, or court order, you should submit your complete application to any tax office in Jamaica. Please view Tax Administration Jamaica (TAJ) or Tax Office contact details via this link:Link
2. Obtain the application form (F7) for a new driver’s license at any office of the Island Traffic Authority (ITA), Tax Office or download an electronic version from the Tax Administration Jamaica (TAJ) website: Link scroll down to find “Form F7 - Application for Driver's Licence Transaction(s)” or you can download the application form directly via this link:Link
3. Complete the form accordingly with the new name that you are using and is on the supporting documentation that proves your change of name and then takes the complete form to a Justice of peace or a Superintendent of police or a member of the attorney to certify the application
4. Obtain the rest of the supporting documents and attach them to the complete application form
5. Pay the relevant updating fee at the tax office and obtain the payment receipt
6. Upon payment submit the complete application to the Tax Office official at the service desk
7. The official will verify your application and issue you an acknowledgment receipt containing your application details
8. The application will then be forward to the responsible authority who will review the application and change the old name to the new name you are using
9. Upon amendment, you will be issued a new driver license with the updated name

Note : You might be required to complete a reading and or a driving test
