Procedure

Apply In-Person:

1. Application form for registering as a veterinary practitioner in Chhattisgarh is available in the Chhattisgarh State Veterinary Council – Link
2. After getting the application form, the applicant has to fill in all the necessary details in the form.
3. Applicant should make sure to carry the originals and photocopies of the documents that are listed under the “Required Documents” section of this page.
4. Applicant has to attach the photocopies of the documents along with the application form while submitting it at the office and a fee for the registration has to be paid. (Please pay fee as mentioned by the authority)
5. The registration certificate will be provided to the applicant within 30 days if the application passes through all the other procedure successfully.

Apply Online

1. Application form for registering as a veterinary practitioner is available in the following link - Link.
2. If the applicant is already a user, they can login with their “User ID” and “Password”.
3. If the applicant is a new user, they have to click on the “Register” button.
4. For new registration, applicant has to enter their Name and other details for registration.
5. Applicant can choose their “Login ID” while registering.
6. After entering all the necessary details for registration, applicant has to click on the “Submit” button.
7. After logging in, applicant has to click on the “New Registration” button.
8. Then, application form for registration will be opened.
9. Applicant has to fill-in all the necessary details in the application form.
10. Applicant has to upload digital copies of the documents (whichever is necessary) that are mentioned under the “Required Documents” section of this page.
11. After entering all the details, applicant must click on the “Submit” button.
12. If all the above-mentioned procedures were followed properly, applicant can get their veterinary practitioner registration within a month.
    
    

Apply through Centres:

1. To Register as a Veterinary Practitioner (Veterinary Council), the applicant shall visit the nearest CSC/Tehsil Centre. Find contacts to CSC centres in the link provided:Link
2. Procure the application form from the concerned centre and fill it up with the necessary details required for the process.
3. Make sure to attach all the "Required Documents" along with the application form before submission.
4. Provide authorized signatory and photograph, wherever applicable.
5. Now submit the application to the office of the concerned centre.
6. On successful verification, the applicant will be notified by the CSC centre.

• Three (3) passport size photographs [recently taken].
• Date of Birth (10th class Mark memo or Matriculation or Secondary Examination Certificate) in Original and 2 Xerox Copies.
• Intermediate Education Certificate in Original and 2 Xerox Copies.
• Provisional Degree Certificate (PDC) in original with 2 Xerox copies.
• Provisional Course Completion Certificate in Original with 2 Xerox copies.
• Provisional Registration Certificate in Original. (Internship Training Certificate) (At the time of Registration, you have to submit your Provisional Registration Certificate in the form of Original).
• Degree Certificate in original with 2 Xerox copies.
• Study Certificate of Degree in original with 2 Xerox copies.
• Aadhar Card Xerox Copy – 2.
