- A copy of the facility's relevant state and federal licenses.
- A copy of any state permissions or authorizations for collection (i.e., aquaculture permit, broodstock permits, and capture logs).
- Records for the prior year's production.
- A signed declaration from the breeder or breeder's record, copies of documents proving that you bought the specimens from the breeder or documentation documenting the history of transactions (chain of ownership of each specimen).

- Application fee of this procedure is
- $100: A single shipment
- $200: Multiple shipments
- Include a non-refundable processing fee in the form of a check or money order made out to the U.S. fish and wildlife service.
- Federal, Tribal, State, and local governments, as well as people working on their behalf, are excluded from the processing cost; please attach proof of your exemption from payment as directed in the instructions.
