Apply In Person
- To Apply for an Affidavit of Service, the applicant has to approach the United States Court link
- NOTE: Search by address, city, state or ZIP Code and click the “Location” button.
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit the documents and completed form to the state or district court.
- The fee for this procedure has to be paid as per the authority’s request.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Once the verification process gets completed, the applicant get notification through mail.
- To complete further process, the applicant has to visit the court on date of schedule to complete further process.
- After all the court hearing process gets completed, the applicant will get the served paper from the court.
- The processing time for this Affidavit takes up to 1 to 2 months or it will be intimated by the court.

- The jurisdiction of the court where the suit is filed
- The case information, including the names of the parties and the case number

Affidavit Purpose
- An affidavit is a formally written statement or document which are sworn and affirmed before an authority who has the power to witness an oath. Affidavits are self-declaratory and are created voluntarily to be used as evidence in courts. The person who creates an affidavit is called a deponent and the affidavit will present facts which are true up to the best knowledge of the deponent.
