Procedure

In person

1. You can apply for change of address in your license by simply going to your local DMV Office within 20 days of the change .
2. Complete and submit a Change of Address Form.
3. Present your current driver's license.

By mail:

You may also complete a Change of Address Form - DMV-98-TRB, attach a copy of your driver's license or state issued photo ID and send it by post to:

Note: No information will be updated without state issued ID or other documentation acceptable to DMV. Incomplete forms will not be returned or updated.

Instructions

You can apply for change of address in your license by simply going to your local DMV Office within 20 days of the change to avoid any possible fine and imprisonment. You failure to change your address on DMV's records may cause you to miss the renewal date of your license, or even have your driving privileges suspended or revoked unnecessarily.

• New Mailing Address
• Drivers License No.
• Last four digits of SSN
• Date of Birth
• Name
