Apply In-PersonEdit

  1. Following are procedures on how you can replace your damaged, lost, or stolen Medicare card in person. First thing you will need to do once you found out that your card was lost or stolen is to report the incident immediately in order to protect your privacy.
  2. Next, you will need to apply for a new Medicare number and card. You can start the replacement process by completing the Medicare Card Replacement Form. A link is provided under 'Documents to Use' section which you can click to download the form.
  3. Print and complete the card replacement form. Make sure that you fill up the form correctly.
  4. Submit the completed forms together with your proof of identity and residency in person to a Service New Brunswick office.
  5. Your stolen card will be terminated and you will be given a new Medicare number and a card will be issued to you upon receipt of the $10.00 fee. It is your responsibility to notify your service providers of your new Medicare number.

  1. To protect your privacy immediately file a report regarding your lost or stolen Medicare Card.

Completed forms must be mailed to Medicare directly together with photocopies of your proof of identity and residency and a cheque or money order, made payable to the Minister of Finance, in the amount of $10.00 for each replacement card required (one card per person only). Send your request to P.O.Box 5100, Fredericton, N.B. E3B 5G8.
  5. Your stolen card will be terminated and you will be given a new Medicare number and card will be issued to you upon receipt of the $10.00 fee. It is your responsibility to notify your service providers of your new Medicare number.

Notes:

  2. The same document may not be used to satisfy more than one requirement.

  • Indian Status Card
  • Baptismal certificate (if place of birth is identified)

Non-Canadians - mandatory:

  • Canadian Immigration Identification Records and Entry Stamp on their Passport
  • Proof of enrollment from an approved learning institution

Documents that may be accepted as proof of identity:

  • Birth Certificate
  • Valid New Brunswick Driver’s License
  • Valid Passport (Canadian or foreign)
  • Baptismal Certificate (if place and date of birth indicated)
  • Valid Permanent Resident Card (front & back) or Record of Landing Document example: (Form # IMM 1000)
  • Active Temporary Resident Permit issued by Citizenship and
  • Immigration Canada
  • Certificate of Canadian Citizenship
  • Canadian Indian Status Card (front & back)
  • Legal Name Change Document
  • Student or Employee ID Card
  • Previous Provincial Health Card

  • Mortgage Document
  • Rental or Lease Agreement (must be signed by landlord & tenant)
  • Utility Bill - not older than 2 months (phone, energy, cable/satellite, water/sewer)
  • Employment Confirmation - current (pay stub or letter from employer on company letterhead)
  • Insurance Policy (home, tenant, auto)
  • Valid NB Motor Vehicle Registration
  • Valid NB Driver's License
  • Child Tax Benefit Statement
  • Property Tax Bill (current year)

EligibilityEdit

You may be eligible for a New Brunswick Medicare coverage if you are:

  • a Canadian citizen or be legally entitled to remain in Canada and a resident who makes his/her permanent and principle home in New Brunswick; or
  • an international student who meets the eligibility criteria.

The following individuals are NOT eligible for coverage:

  • regular members of the Canadian Armed Forces, (however members of their families are eligible);
  • tourists and visitors to the province;
  • transients;
  • students from another province; and
  • inmates of federal penitentiaries.

Notes:

  • A “resident” means a person lawfully entitled to be or to remain in Canada. A person who makes his home and is ordinarily present in New Brunswick, but does not include a tourist, transient or visitor to the Province; as legislated under New Brunswick's Medical Services Payment Act.
  • Maintaining a dwelling, owning property in the province of New Brunswick, or paying New Brunswick property or income tax does not mean you are eligible to avail insured services under New Brunswick Medicare.

FeesEdit

Replacement fee: $10.00

Payment may be in the form of a cheque or money order, made payable to the Minister of Finance, in the amount of $10.00 for each replacement card required (one card per person only).

Individuals who are exempt from the $10.00 replacement card fee:

  • Seniors who receive the Guaranteed Income Supplement,
  • Income assistance recipients, and
  • Resident’s who’ve received a damaged or misprinted card.

ValidityEdit

  • Cards may be renewed for five years.
  • Your card is valid until the last day of the month indicated. For example, a card showing the date "03/2020" is valid until March 31, 2020.
  • Provided you are eligible for coverage and have kept Medicare informed of your current address and/or residency information, you may receive your card approximately 1 month before this date. Destroy your old card and use the updated card effective immediately.
