Counter service is available at 254 Portage Avenue, Winnipeg.
The building is located at the SW corner of Portage Avenue and Garry Street.
The Agency is open Monday through Friday, from 8:30 A.M. to 4:30 P.M.
Available payment methods are cash, cheque, money order, debit, Visa or MasterCard.
Please note personal cheques, unless certified, will NOT be accepted for 24 hour RUSH service.

The information requested on the Application Form is collected pursuant to The Vital Statistics Act to fulfill the requirements for the release of birth, marriage and death information.

Any person with a valid reason.
Only next-of-kin may obtain a certified photocopy of a death registration.

For each certificate issued by the director, including the search: $30.00

InstructionsEdit

All the appropriate sections of the application form must be completed in full.
Incomplete applications or those with insufficient payment will be returned.
Documents can be issued only if the birth, marriage or death occurred in Manitoba. Certificates contain information extracted from the original registration filed at our office.

Need for the DocumentEdit

A death certificate can describe either a document issued by a medical practitioner certifying the deceased state of a person or popularly to a document issued by a person such as a registrar of vital statistics that declares the date, location and cause of a person's death as later entered in an official register of deaths.
