ProcedureEdit

  • This form is usually completed at the funeral home when the funeral arrangements are being made. *The funeral home will ensure the original documents are sent to Vital Statistics for processing.
  • Once a death is legally registered, corrections or amendments may only be completed with the appropriate documentation and affidavits.
  • For information or clarification on the death registration process, contact the Vital Statistics office. Funeral home staff may also be able to provide some assistance.

EligibilityEdit

It is the responsibility of the spouse, next of kin, or person who has full knowledge of the facts surrounding the deceased person's death to complete a Registration of Death form.



FeesEdit

If an amendment is requested within 90 days from the date of event (birth, marriage, death), there is no fee. However, after 90 days, a $20.00 government fee will be applied.
