- You can request for a copy of a death certificate in person or you may also send your request via email (see contact information below). Note: For request send via email, it will be better if you will give the registry office a call to check how you can arrange payment for the certificate or other required information.
- When you submit your request make sure that you have provided the following details:
- name of the deceased
- date of death
- place of death
When Living Outside the CountryEdit
It may be possible for you to obtain a copy of a death certificate from a Cook Islands overseas office or diplomatic representatives of the Cook Islands in foreign countries, you may use the link below under 'Office Locations & Contacts' section to see a list of addresses and contact details.

EligibilityEdit
Only close members of the family can apply for a death certificate.
FeesEdit
- For every certified copy of an entry in a register book kept in a Registrar's office, and for every certified copy of an entry in the office of the Registrar-General (when correct year is given): $25.00
- For every certificate of date of death when correct year given, including any certificate issued pursuant to section 44 of the Act: $15.00

InstructionsEdit
The Registry Office only records deaths within the Cook Islands.
By law, deaths must be registered within three (3) days of burial. If the body is missing, registration can only take place after seven (7) years. For un-natural deaths, you will have to wait for the Coroners' report before the registration can proceed.
Once a death is registered, a death certificate will be issued.
