Municipalities and Public Institutions

• Subscription Application Form convention.
• Photocopy RUT, name and address of the municipality or institution.
• Decree of appointment of the Mayor or Head of the Institution higher.
• Photocopy of identity card of the legal representative of the Municipality (Mayor) or upper head of the institution that acts as legal representative.
• Photocopy of identity card of the coordinator of the agreement, the position within the Municipality or Institution.
• Letter expressing reason, object and purpose for requesting the execution of the agreement.

Private entities

• Subscription Application Form convention.
• Photocopy RUT, name and address of the company.
• Legalized photocopy of the deed of incorporation.
• Copy of registration in Commercial Register extract incorporation.
• A copy of the publication in the Official Journal of the extract of incorporation.
• Certificate of validity issued by the conservative society of real estate.
• Copy of the deed by which powers are granted to legal representatives of the company.
• Certificate of validity of the powers, issued by the Real Estate.
• Photocopy of the identity card or the legal representatives, with his position in the company.
• Photocopy of identity card of the convention coordinator, with his position in the company.
• Letter expressing reason, object and purpose for requesting the execution of the agreement.

The values ??that the agreement signed Entity must be paid for the provision of information services are established by Exempt Resolution No. 2357 dated October 1, 2004, the Civil Registry and Identification.

Thus, the values ??that the Bank shall pay for the services described in this agreement are:

The replacement service connection to the corporate network of the Civil Registry and Identification will be priced 2 UF every time you perform. All prices shown must adicionrseles the Value Added Tax (VAT), setting the final price 5 decimals. Payments will be made ??in the national currency equivalent, as the current value for the Unidad de Fomento (UF) to the date specified in the respective agreement.

Validity
