To purchase a certified copy of a Utah vital record and pay with a credit card, whether online, by phone or fax, you may go through VitalChek's Online Ordering System.
- Order Online: Go online and simply follow the steps to place an order. An additional fee is charged by VitalChek for using this service and all major credit cards are accepted. You will receive an e-mail notification once your certificate is shipped. If your shipment is via UPS, you will also receive the UPS tracking number.

InstructionsEdit
- The Utah Department of Health, Office of Vital Records and Statistics (Utah Vital Records) issues certified copies of Utah death certificates for events which occurred within the State of Utah.
- Death Certificates are available for events that occurred within the State of Utah from 1905 to present.
- Certificates for deaths that occurred in Utah since 1905 are on file in this office. If the death did not occur in Utah, please visit NCHS - Where to Write for Vital Records.
- You may request for copies of vital records online, by phone, by mail and in person.
- Check your certificate for accuracy. Your copy can only be replaced within 90 days from issuance date. After 90 days you must repay applicable fees.
- If requestor does not respond to a written request for information within 90 days, Vital Records may retain all monies paid.
