Procedure

Apply In-Person:

1. The application form for National Trust Fund (GNTF) forms is available in the executive secretary in National Pensions Regulatory Authority office in Ghana contact link
2. Applicant should obtain the application form from the respective office.
3. Applicant has to enter all information in the application form.
4. Send a letter of application to National Trust Fund secretariat specifying your level of disability and the financial support applicant need from the Trust. This letter should be addressed to the Executive secretary National trust fund.
5. The next step is to include an all the required document, proof of disability a full photography and a medical letter can work here, also include recommendation letters from and any other supporting documents before submission.
6. The applicant has to make sure he or she has all the required documents that are required for this process to be successful and they can be found under the "Required Documents" section of this page.
7. Payment of fees for this procedure has to be paid as per as authority’s request.
8. Submit the applicant application to the executive secretary of the Trust for the Board approval, all applications are approved by the board after short listing is done, and once approval has been done payment is affected.
9. Generally, the processing time for this procedure 6 weeks.

• Any Disabled person in Ghana and Community organizations supporting disabled persons in Ghana are eligible.
• The applicant must be Ghanaian and must be Handicapped.

• Payment of fees for this procedure has to be paid as per as authority’s request.

Validity

• Valid for 1 year and it is given once.

• The Social welfare officers in the Regions are entrusted with the responsibility of monitoring donated equipments in the region.
